1. The provider is OON (out of network) for the insurance plan you are covered under
2. Your insurance plan was inactive at the time of service, or is currently inactive
3. The services claimed are not covered by the insurance company
4. The insurance company has determined that the services claimed were pre-existing and therefore ineligible for coverage
